As much as we all hate it (seriously; is there anyone out there who’s completely unbothered by the presence of a new zit or two?), there’s no denying that it’s as natural as breathing.
The sun rises in the east, spring comes after winter, and every human being alive has experienced getting pimples.
Just facts, you know?
So let’s keep things real by inflicting the same physical and emotional stress onto your Sims with these realistic custom acne sets.
1. Acne Skin Detail Redux by seaslugsims
It’s kind of hard to describe the different variations of seaslugsims’ Acne Skin Detail pack, so I’ll let the preview photos do the talking.
All you need to know is that they add acne marks and scars in the usual locations: nose, chin, forehead, and the occasional cheek.
It’s subtle but realistic, which makes it one of my fave sets.
2. Real Skin Acne by sorokacc
Sorokacc’s Real Skin Acne has 21 swatches, making it one of the more extensive custom skin details on this list.
Like seaslugsims’ acne set, the marks here are subtle but still visible – like actual acne. It’s not non-existent, but it’s also not exaggerated. It just turns a Sim’s otherwise butter-smooth skin into something textured and closer to real life.
3. Acne #01 by Scarlett-content
If you’re looking for some pretty intense acne scarring – like the kind fresh from a breakout – Scarlett-content’s Acne #01 fits to a T.
The marks are red, rough, and look like they could use a few more days to heal.
You’ll find it in the Makeup category, under Blushes.
4. Acne #02 and #03 by Scarlett-content
I decided to combine the creator’s Acne #02 and Acne #03 CC because of how similar they are in terms of placement and intensity.
They have that same rubbed rawness that Acne #01 has, only they aren’t as freshly cracked.
These custom skin details (still found in makeup) look like they’re well on their way to being treated.
5. Zits, with Two Levels of Intensity by Around the Sims 4
ATS4’s custom zits are probably the closest match to unmodded EA skin details.
They’re visible and exaggerated, but in a way that’s acceptable – and actually expected! – by Sims 4 visuals.
There are three variations in terms of placement, and each variation has two intensities. This level of customization makes it easy to overlay the acne over different skin colors.
6. Acne Prone Mask by mellouwsim
This Acne Prone Mask has four swatches—one for each intensity.
You can’t change the placement or look of the acne (and acne scars), but you can adjust the healing stage.
And because it’s a Skin Detail, you can freely overlay other makeup or face paint over it for more texture!
7. Flora Acne by MSQSIMS
Like mellouwsim’s Acne Prone Mask, this Flora Acne has a fixed placement but different intensities. Nine, this time around!
There are about three levels of acne scarring, and each level can be adjusted to blend better with light, medium, and deep skintones.

8. Acne Scars #3 by xrattrap
These custom acne scars by xrattrap are a lot more subtle than the other skin details on this list.
They’re more scabs and pits than they are freshly-popped and healing pimples.
This is to say, they’re more rough and textured than they are red and shiny. However, they’re still plenty visible when slapped on Sims 4 skin!
9. Better Zits by thriftytrait
If you have The Sims 4: Parenthood installed, these zits by thriftytrait can override EA’s acne (that came with the pack).
If you don’t, don’t fret!
The creator offers a BGC version of their edited pimples that you can access – and use – through the Face Details and/or Makeup category.
10. Back Acne by seaslugsims
As I’m sure you know, acne isn’t exclusively for the face.
Some people suffer from “backne”—aka zits that appear on your back.
I mean, pimples are generally a whole-body affair. People just tend to forget since face acne is the most prevalent and instantly noticeable.
But if you’d like to add just a bit more realism to your game, seaslugsims’ Back Acne could be worth playing around with.
